This position provides comprehensive social work, medical case management, support and referral services to primarily young adults (age 17-24) and women with HIV infection receiving medical services in the UNC Infectious Diseases Clinic. A smaller percentage of time will involve provision of services to adult HIV patients and patients with other infectious diseases in collaboration with other members of the social work team. Selected candidate will work closely with medical providers and other interdisciplinary team members including nurses, medical providers, pharmacists, and benefits coordinators. Medical case management includes the provision of treatment adherence counseling to ensure readiness for, adherence to, and persistence with HIV therapy. Key activities include: (1) initial assessment of service needs; (2) development of comprehensive, individualized care plan; (3) coordination of services required to implement the plan; (4) client monitoring to assess the efficacy of the plan; and (5) periodic reevaluation and refinement of the plan, at least every 6 months, as necessary. It includes client-specific advocacy and review of service utilization. A variety of skills for case management are used, including face-to-face meetings, telephone/video calls, social media, text messaging and any other forms of communication. Selected candidate must display excellent communication skills on a variety of platforms. Selected candidate will be communicating with clients, partners, families, and their support systems, as appropriate to support the treatment plan. Selected candidate will intervene to facilitate linkage, engagement and retention in medical care through individualized and systematic interventions. They are expected to provide mental health and substance use assessment and interventions and to coordinate services with other community resources. They may conduct HIV testing, after receiving necessary training. Position requires timely data entry and data management in the electronic medical record and other databases. Candidate must demonstrate a strong ability to multi-task in a fast-paced environment. Tracking of service numbers and creation of periodic reports is expected. Position will assist in the development of innovative programs, policies, procedures and protocols to meet the needs of the clinic population. SW will participate in and lead clinical quality improvement activities. Position is expected to provide coverage for SW colleagues as needed and attend monthly staff and quarterly provider meetings and other meetings as directed.
