JOB SUMMARY: The Social Worker conducts psycho-social evaluations, provides individual, family, and group counseling services to consumers and coordinates treatment with medical clinic staff for the Intake and Counseling Program.

ESSENTIAL DUTIES and RESPONSIBILITIES:

Provide individual adults, family, children, youth and/or group therapy.
Establish therapeutic relationships with consumers to promote acquisition of rehabilitative goals.
Maintain client documentation according to required policies and procedures including assessments, contact notes, and any and all other clinical documentation related to the care of the client.
Link consumers to resources in the community as needed.
Provide information and referral services to consumers’ families, as appropriate and necessary.
Provide crisis intervention, as necessary.
Consult with and coordinate client treatment with psychiatric consultants, clinic providers and other support staff
Meet productivity standard of 5 hours of billable services average per day.
Maintain good working relationships across departments and with external customers (Primary Care Coalition, Immigration Legal Services, local medical clinics, etc).
Maintain a high standard of organization; operate independently as a licensed clinician.
Make recommendations to the Medical Director and/or Quality Improvement staff related to process improvement, based on best-practice models.
Supervises, and accesses work of intern(s) if assigned.
EDUCATION and EXPERIENCE:

Master’s Degree in Social Work.
LMSW in MD and willing to get a DC LGSW, LICSW preferred.
3 years of direct service experience in a mental health setting.
Experience providing counseling to adults with serious and persistent mental illness, many with co-occurring substance abuse issues and histories of trauma.
Experience diagnosing.
SKILLS and COMPETENCIES:

Communication skills (written and verbal).
Bilingual, English-Spanish. (Fluent in speaking, reading and writing English and Spanish).
Excellent interpersonal skills Assessment/analytical skills.
