The Transforming Health and Resilience in Veterans (THRIVE) Program is seeking a full-time Social Work Practitioner/ Case Manager. The THRIVE Program is a community-based clinical outreach program designed to provide care to veterans and first responders who are experiencing the effects of traumatic brain injuries or post-traumatic stress, resulting from a career serving our nation. The program will evaluate, diagnose, and treat physical, cognitive and other health conditions during a 3-day evaluation and Intensive Outpatient Program (IOP).

This position will work within a comprehensive interdisciplinary medical team to assess patient needs. The Case Manager will provide screening interviews to identify at risk social and behavioral patients entering the THRIVE Program. The position will coordinate referrals to resources within the community to further address medical, mental health, behavioral health, and social needs of the patient. When identifying at risk patients, this position must be able to make critical decisions regarding the patients’ mental/physical health needs to include enhanced social work assistance.
