This position provides comprehensive social work practitioner (SWP) services to a high volume of adults with HIV infection receiving medical care in the Infectious Diseases Clinic. A small percentage of time involves provision of services to patients with other infectious diseases. Direct HIV social work services will include a comprehensive assessment of needs, medical case management, mental health and substance use screening and services, and linkage of clients to health care, psycho-social, and other resources.
The SWP will facilitate linkage, engagement and retention in medical care through individualized and systematic interventions. SWP will work collaboratively with an interdisciplinary team of medical providers, nursing staff, benefits coordinators and other staff. SWP is expected to coordinate services with other community resources. SWP may conduct HIV testing with training. Position requires timely documentation in the electronic medical record and data entry into other databases. The SWP should be able to multi-task in a fast-paced clinical environment. Monitoring service delivery including creation of periodic service and project summary reports is expected. SWP will assist in the development of policies, procedures, and protocols to meet the needs of the clinic population. SWP will participate in clinical quality improvement activities. SWP will work with other SWP colleagues to ensure clinic coverage and attend and present at grant and staff meetings. Role requires direct supervision of other social work staff including orientation and training. It may include clinical supervision for other clinical social workers to obtain licensure and certification.
