Medical Social Worker

Living Through My Dreams, LLC was established in 2019 and prides itself on providing quality service to children and families who are in need of support from Medical Social Workers who will go the extra mile and help support families with resources that will enhance their lives. Living Through My Dreams, LLC is looking for energetic, graduates and soon-to-be graduates to join our team. We provide unique and tailored experiences to the families we service and believe in building lasting relationships with the children, families, and staff that become a part of our family. Responsibilities

• Maintain a safe and hazard-free work environment by following policies and procedures of Living Through My Dreams Home Care, LLC
• The Social Worker shall provide education and resource identification while providing quality service with cost reduction in mind.
• The Social Worker will obtain a daily list of patients with no PCP and will facilitate the process to identify and obtain a PCP for this patient population
• Guide and educate family members and caregivers
• Basic computer experience (Microsoft Office, Internet, Electronic Medical Record, etc)
• Excellent interpersonal communication skills

Qualifications:

• Bachelor’s degree in Social Work or related field from an accredited college or university with a minimum of 1-2 years experience in social work. Certification preferred and MSW preferred.
• Extensive experience preferably in community.
• Thorough knowledge in principals and methods of social work and counseling.
• Advanced skills in negotiation with State, Federal and community facilities and agencies. Knowledgeable of State and Federal laws as they relate to social work reporting practices.
• Exemplary verbal and written communication, creative thinking and cost savings practice.
• Assists the patient and significant others to understand, accept, follow, and implement medical recommendations.
• Be an active team player that contributes to the culture where everyone thrives.
