We are currently seeking a dedicated Social Worker to provide intensive Case Management Services and group leadership to the guests that access the South Wilmington Street Center (SWSC). The focus of this position is to assist individuals needing our services to transition into permanent housing in a timely manner. The Social Worker will be conducting two groups each week for guests that are enrolled in the Progressive Housing Program (PHP). Review of each guest’s progress during the past week and extending the guest’s time in the program for another week if progress warrants an extension. The Social Worker will inform guests of training and educational opportunities offered in the community as well those being offered at the SWSC.

This position works 8hr shifts Monday thru Friday; Hours are 8:30am – 5:15pm (3 days) 12:30pm – 9:15pm (2 days); staff works one (1) weekend day based on a rotation among social works on the team. This usually occurs once every 5-6 weeks.

As the population and economy continue to grow in Wake County, the need for affordable housing development and preservation increases. Not only do more units of affordable housing need to be built, housing services must be provided to support families and individuals to access or maintain safe, decent, affordable housing. Affordable housing is also critical to preserving Wake County’s economic competitiveness by offering housing for workers at all income levels, supporting housing stability and economic opportunity for its residents, and furthering Wake County’s commitment to healthy and inclusive growth. The Department’s programs include: Community Revitalization, Development Financing, Homeless and Prevention Services, Emergency Shelter, Support Services and Permanent Supportive Housing.

Essential Job Functions;

*Facilitate group meetings each week with a specific population and meet individually with guests as needed to discuss specific needs and provide the information needed to address those needs
*Meet daily with the Case Management Team to discuss guest’s progress in the PHP program, and address any concerns with guest’s progress
*Make monthly home visits with guests that have moved out of the SWSC that require Case Management Services. Visits may require more frequent home visits to provide that level of care needed to assist guest in maintaining housing
*Provide Individual Intensive Case Management Services including developing individual Service Plans that will include a housing goal, develop Budgets with guests, complete applications for housing for guests and provide transportation for guest when needed to help them keep scheduled appointments
