We are currently seeking a Housing Social Worker to become a member of an interdisciplinary team. The Housing Social worker acts as a SSI/SSDI Outreach, Access, and Recovery (SOAR) Benefits Specialist and will work with eligible Permanent Housing and Supportive Services program participants in obtaining benefits/income. SSI/SSDI Outreach, Access, and Recovery (SOAR) is a model that helps individuals experiencing or at risk of homelessness who have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder apply for Social Security disability benefits. This position will report to the Rental Assistance Housing Program (RAHP) Supervisor.

The essential functions of the Housing Social Worker are as follows:

*Working with the Permanent Housing Supportive Services (PHSS) teams, referral sources and community partners to identify applicants and complete interviews with individuals to gather information to complete SSI/SSDI applications
*Writing SOAR Medical Summary Reports for individual applications
*Assisting Social Workers gather medical records and other information to complete SSI/SSDI applications
*Accompanying individuals to appointments at the Social Security Administration
*Assisting program staff to coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the application. *Transporting clients as necessary
*This position may perform other duties in addition to the above duties as assigned by management.
