This position is responsible for planning and implementing a plan of care in accordance with the patient’s wishes to address the patient and family’s emotional, social, legal, financial, spiritual and practical needs in order to affect satisfactory outcomes for coping with the patient’s illness. This position is a 40-hour a week opportunity in an inpatient hospice home.
