The Orange County Health Department has an opening for a Health Social Worker II. The work involves the responsibility for providing professional social work services for the Care Management for High Risk Pregnancies (CMHRP) program and the Care Management for At-Risk Children (CMARC) program with the Orange County Health Department. An employee is this class is responsible for provided services which include assessment, intervention, client education and high level comprehensive and intensive care management services to a limited caseload of eligible at-risk pregnant women and high-risk children (ages 0-5 years) with complicated personal, psycho-social, medical, developmental and economic needs.

Work is performed under the general supervision of the Home Visiting Services Supervisor with some leeway to exercise independent judgment within the framework of applicable rules, regulations, policies and procedures in the performance of duties, both in terms of planning and accomplishing work and in making decisions about public health needs and requests. Supervision is not a responsibility of this class. Does related work as required.

Examples of Essential Functions
Following are examples of typical work activities and responsibilities. The incumbent may be responsible for performing other related duties and responsibilities as required or assigned. A position may not include all the work examples given, nor does the list include all that may be assigned.

Conducts comprehensive health and risk assessments and provides Care Management for High Risk Pregnancy (CMHRP) and Care Management for At Risk Children (CMARC) program services to at risk Medicaid eligible pregnant recipients and high risk children (ages 0 to 5 years) and their families; assessments and care can be provided in the home, clinic, provider offices or by phone.

Completes medical, psycho-social, developmental, economic and environmental assessments and evaluations, and implements intensive, face-to-face care management services for at-risk patients with serious needs.

Reviews patient medical record documentation to assess needs and develop a plan of care; develops patient centered care plans to address patient and program goals.

Provides Triple P (Positive Parenting Program) parenting interventions.

Provides counseling utilizing motivational interviewing, problem solving, crisis management and conflict resolution techniques.

Provides ongoing care coordination, monitoring and evaluation of effectiveness of interventions; and uses the electronic to access reports, hospital data and to complete documentation.

Provides education on prenatal care, child development, depression, postpartum care, family planning and the prevention and management of serious or chronic health-related conditions.

Identifies and coordinates needed support services by initiating referrals to community resources to strengthen the family unit to assure improved family functioning and positive health outcomes.

Acts as consultant/resource person to other professionals, and serves as an external liaison between client and community resources.

Works collaboratively with a multidisciplinary community team including OB/GYN and Primary Care physicians and providers, and is actively involved in the effective exchange of information.

Attends community team meetings with the court system and the Department of Social Services to develop care plans and review compliance toward identified goals.

Attends meetings and conferences for the purpose of providing information about service goals and objectives, and to provide consultation on the special needs of clients.

Documents all care management activities in an electronic medical record system; ensures that proper documentation is completed according to State policy guidelines.

Uses computer applications or other automated systems such as Electronic Medical Record and), spreadsheets, word processing, calendar, e-mail and database software in performing work assignments.

Adheres to the department’s safety rules and regulations.

FULL PERFORMANCE OF KNOWLEDGE, SKILLS, ABILITIES
Comprehensive knowledge of the principles and practices of public health, public health nursing, and social work.

Comprehensive knowledge of the federal, state and local laws, ordinances, standards and regulations regarding public health, child abuse and neglect.

Thorough knowledge of the federal and state program rules, procedures, protocols, and policies relating to service provision.

Thorough knowledge of current social, economic and health trends and services, and the ability to adapt trends to practical program applications and link clients with services needed.

MINIMUM QUALIFICATIONS
Graduation from an accredited college or university with a Bachelor of Social Work (BSW, BA in SW, or BS in SW) or Master of Social Work (MSW, MA in SW, or MS in SW) from a Council on Social Work Education (CSWE) accredited social work degree program; OR

A Registered Nurse, degree in nursing and licensed to practice in NC; OR

Graduation from an accredited college or university with a Bachaelor’s Degree in a human service field with five (5) years of care management/case management experience working with the specific population of low-income, pregnant individuals and/or children ages 0-5 years; OR

Graduation from an accredited college or university with a Bachaelor’s Degree in a human service field with three (3) years of care management/case management experience working with the specific population (low income, pregnant individuals and/or children ages 0-5 years) and certified as a Case Manager (CCM preferred)

Previous work experience in home visiting and case management and collaborating with medical homes and medical providers is preferred.

SPECIAL REQUIREMENTS
Possession of or ability to readily obtain a valid Driver License issued by the State of North Carolina for the type of vehicle or equipment operated.

Possession of or ability to obtain CPR and First Aid certification within 6 months of date of hire.

All offers of employment are conditional upon successful clearance of a background check and pre-employment drug testing. Employment cannot commence until the employee has been cleared.

May be required to possess additional certification(s) as deemed necessary by the Home Visiting Services Supervisor.

PHYSICAL REQUIREMENTS AND POSSIBLE HAZARDS
Ability to physically perform (with or without reasonable accommodation) basic life operational functions of climbing, balancing, stooping, kneeling, crouching, crawling, reaching, standing, walking, pushing, pulling, lifting, grasping, feeling, talking, hearing and repetitive motions of the hands/wrists.

Must be able to perform light work exerting up to 20 pounds of force occasionally and/or up to 10 pounds of force frequently and/or a negligible amount of force constantly to lift, carry, push, pull, or otherwise move objects.

Must possess the visual acuity to prepare and analyze data and figures, perform extensive reading and operate a computer terminal.

May be subject to possible hazards that include emotional stress due to the nature of interactions with clients or service providers, irate clients and/or family members who may be hostile, resistant or violent.
