The primary purpose of this position is to provide service coordination for children in the NC Infant-Toddler Program and to provide supports to their families/caregivers. Service coordination is a critical service for families/caregivers and it is required under the Individuals with Disabilities Education Act (IDEA) Part C. Services provided through the NC ITP must be compliant with the IDEA, federal regulations, and State policies and procedures.

This position as an Early Intervention Service Coordinator for children ages birth to three is located within the Children’s Developmental Services Agency of Western North Carolina. Duties include participation in developmental evaluation and assessment to determine eligibility for the ITP, developing functional Individualized Family Service Plans (IFSPs), coordinating with multi-disciplinary teams to determine families’ needs for supports and services, conducting home visits, coordinating and facilitating ITP services and transition planning, and participating in the authorization and monitoring of early intervention services.
