General Summary
As an integral part of the DA-NC team, the Dementia Navigator provides support to caregivers and families on issues related to caregiving and dementia. This person determines the specific needs of individual families and as a result, provides personalized information, referrals, and other forms of caregiver assistance. This will be through phone, online and in person consultations.
Essential Job Functions:
Provide compassionate and effective support, education and assistance (according to scope of practice) to individuals living with dementia, families, caregivers and the community through needs assessment, disease education, information and referral, supportive individual and family counseling and follow up.
Collaborate with other service professionals to assist families in accessing services that enable them to care for an individual living with dementia or themselves.
Attend meetings, conferences and workshops to promote DANC programs and services and stay on top of NC caregiver needs.
Remain informed about current research and information relevant to the fields of dementia and caregiving.
Help develop and maintain high-quality documentation including database entry, metrics and providing impact reports as needed. Compile and report on all statistical data required for internal and external management of services and grant funds.
Promote the Dementia Navigation program through networking, committee participation, writing a variety of materials including social media and blog posts, newsletter articles, and website additions, and collaborating with other agencies and foundations.
Provide partnership and leadership in the coordination, delivery and evaluation of therapeutic programs and services including but not limited to support groups, early-stage dementia programs and education classes.
Work within a close team to support the efforts of each position, especially assisting in fundraising and education events.
Facilitate ongoing community engagement programs.
Develop new and maintain existing relationships with community partners.
Facilitate community support group(s) and assist with Support Group Leader training.
Develop and update materials and resources for family caregivers.
Conduct training and informational presentations.
Other Duties as assigned.
Qualifications, Skills and Experience Required:
MSW in Social Work strongly preferred; BA in Social Work or equivalent work experience in social work, gerontology, counseling, or a related field.;
2-5 years of experience working in aging or a dementia-related position.
Excellent knowledge of dementias, caregiver issues, NC aging community resources, social work principles.
Excellent interpersonal skills, personable and able to work with people living with dementia, care partners/caregivers, family members and professionals in a wide variety of fields and from diverse backgrounds.
Demonstrated commitment to valuing diversity and contributing to an inclusive working and learning environment.
Excellent oral and written communication and customer service skills, demonstrating professional and effective communication by phone, email and in person.
An awareness and high-level of professionalism regarding sensitive and confidential information. High level of discretion, good reasoning abilities and sound judgment.
Well-organized, self-starter who is also a team player. The ability to prioritize and accomplish day-to-day tasks and long-term goals and to identify needs appropriate to the role and fill them.
Creativity and innovation to solve problems and overcome challenges.
Must be proficient in case management software, Microsoft Word, Excel, and PowerPoint and G-Suite programs.
Possession of a valid North Carolina driver’s license and access to a car on a regular basis and an ability to travel across the state as needed (Travel reimbursed by organization).
Knowledge of database software computer programs preferred.
Presentation and public speaking skills in related subject matter are a bonus.
Being bilingual is a bonus.
Experience leading support groups is a bonus.
Hours & Schedule:
Full time
Office hours: 9:00 a.m. until 5:00 p.m.
Occasional evenings and weekends required
Some travel throughout North Carolina is required.

Salary based on experience and starting at $50,000
