POSITION SUMMARY: Under the supervision of the Director of Case Management, the Community Case Manager is responsible for providing a case management program to families within the Y Excel programs. They will also serve as a role model for them by exemplifying high standards and values. They are responsible for managing those functions in accordance with the stated mission, goals and policies established by the administrative staff, governing committees, and Board of Directors of the YMCA

ESSENTIAL FUNCTIONS:

Case Management 

Identifies and maintains a caseload of 10-15 families at a time that need additional support, which follows a case management plan for short-term intervention (Around 6 months of meetings).
Develops and maintains positive relationships with community providers, volunteers, clients, program participants, and colleagues at all levels of the organization; helps individuals and families connect with each other and the YMCA through motivational support and guidance; and encourages involvement with community support resources.
Meets with families on a regular basis in their home or at the YMCA
Connects the family to additional community resources including but not limited to: financial support, school personnel, immigration services, food pantries, mental health services, physical health services, etc.
Develops or acquires curriculum to deliver to families as additional needs come to our attention.
Maintains proper records; prepares required reports as directed, as well as any assessments, intake or exit interviews, data input, record keeping, or reporting required by funders and relevant state or other regulatory authorities. Participates as requested in audit and quality assurance activities.
Creates a transition plan for each case management family as they complete the program
Through the summer months, come up with specialized programming to offer camp families
Continuously engages creatively in ways to implement and achieve the 5 Pillars of the SE Raleigh Purpose Built Community in all program areas
Develops and maintains local partnerships
Works with the school administration to connect parents to their child’s school
Coordinates events in conjunction with school staff to promote school led initiatives and supplement the school with any additional programs families ask for
Connects with local church and nonprofit groups and continue to expand the network of services available to families we serve
Leads the quarterly Hispanic Ministries meeting

Parent Council Group

Supports a group of parent leaders that meet monthly, in partnership with the Director of Case Management
Stays in contact with the members of the group on a monthly basis. This can consist of a phone call or home visit
Encourages and train parent leaders as they lead subcommittee groups around their expressed areas of interest

Additional Responsibilities

Helps the branch outreach team communicate to families, promote additional events, and translate materials
Maintains all necessary job trainings
Attends YMCA meetings and trainings as determined by Director of Case Management
Works in partnership with Director of Case Management in attending pertinent community meetings, trainings, and conferences

QUALIFICATIONS:

Bachelor’s degree in related field preferred and/or 1-2 years of related experience.
Bilingual in Spanish and English is required
This person must be at least 21 years of age and must be a positive role model for children
Experienced in and passionate about creating and fostering communities and relationship building through programs
Ability to think strategically and creatively about finding solutions to case load’s areas of need
Flexibility in work schedule which will include some evening and weekends as needed
Comfortable traveling and meeting with families inside their home
Exceptional oral, public speaking and written communication skills
This position requires an understanding of family systems, working collaboratively, and culturally competent case management model.

While performing the duties of this job, the employee is regularly required to use a computer for extended periods of time and be able to communicate using a computer and phone/smart device.
The employee frequently is required to sit and reach and must be able to move around the work environment.
The employee must occasionally lift and/or move up to 40 pounds. 
Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust.
The noise level in the work environment is usually moderate.
Ability to obtain upon hire certifications in Basic Life Support, Basic First Aid, Emergency Oxygen and Emergency Medication Administration Training. All certifications must stay current while employed with the YMCA.
A Commercial Driver’s License, with “P” endorsement must be obtained with-in the first year of employment in order to drive a YMCA bus as needed.
