Job description / responsibilities for this position:
This position is located in the Care Management Department and will be working primarily with Adult Cystic Fibrosis patients in an outpatient setting. The Clinical Social Worker completes bio-psychosocial assessments, supportive therapy, provides information and short term interventions to patients and their families in relation to social, psychological, financial and family situations. The Clinical Social Worker obtains, interprets, and communicates to multi-disciplinary team necessary information regarding patients and families, including bio-psychosocial history information. The Clinical Social Worker identifies possible or actual patient/family crises, provides crisis intervention and emotional support, and facilitates effective plans for resolution. The Clinical Social Worker develops and maintains good working relationships with outside community health and social agencies. The Clinical social worker reaches out to the community to learn of and utilize new resources for meeting patient needs. The Clinical Social Worker assists patients and families with obtaining community assistance by referral to proper resources. LCSW preferred.
