Located in nearby Danville, Virginia, Southern Virginia Mental Health Institute, a 72-bed Joint Commission accredited State Mental Health Facility, is seeking an energetic, clinically skilled, organized individual to provide a range of casework activities related to a comprehensive needs assessment involving each patient’s discharge planning process for psychiatric patients in an inpatient setting. This position will serve as a Case Manager, providing an array of services including Case Management, psychosocial assessments, and social work services on the unit and may serve on department and facility committees. The Case Manager will coordinate the discharge planning needs of assigned patients, with treatment team and CSB staff.
