JOB SUMMARY: The Navigator Case Worker provides comprehensive, holistic support services to vulnerable immigrants, assisting them in identifying and accessing needed services. The position works with clients to assess needs, develop a plan, and ope-rationalize the plan, leading to positive social outcomes.
ESSENTIAL DUTIES and RESPONSIBILITIES:
Perform individual casework through:
Conducting needs assessments with clients and developing goal plans in accordance with the needs and wishes of the clients.
Making referrals to providers and assisting clients in connecting with providers for benefit programs, support services, and training and educational opportunities.
Assisting clients with achieving their short-term goals related to self-sufficiency and acclimation.
Collaborate with service providers and advocate for clients as needed.
Maintaining prompt and regular contact with clients in the ongoing provision of services.
Conducting monthly follow-up with clients for up to 30 days then the case worker will assess and provide additional 60 days if warranted.
Collaborate with programs and teams that support newcomers.
Actively participating in team meetings, reporting on the progress of cases, and working with colleagues to problem solve and develop solutions.
Working with Immigration Legal Services staff to support clients in action plans that will lead to positive legal outcomes and those relate their own goals as well as that of their families.
Provide training and education to clients.
Engage in group work to support a range of topics of particular interest to newcomers.
Documenting attendance and participation according to agency protocols.
Conduct outreach through:
Cultivating and maintaining productive relationships with community contacts including but not limited to government and non-government agencies, parishes, schools, and medical providers.
Attending community meetings and outreach events.
Supporting recruitment efforts for volunteers and targeted service providers.
Document outputs and outcomes.
Promptly administering initial assessment instrument and periodic follow up assessments and entering scores in the database.
Entering and maintaining client data into designated computer systems and maintaining client files (paper and electronic) in accordance with policies and procedures.
Tracking all referrals and their outcomes.
Timely completing and submitting qualitative and quantitative reports as required.
EDUCATION and EXPERIENCE:
AA degree.
Three (3) years’ experience providing services to vulnerable populations.
SKILLS and COMPETENCIES:
Strong spoken communication skills in English and in Spanish.
Driving is required. Must have a valid driver’s license and clean driving record.
