WORKING HOURS: Monday – Friday with some weekend work

ANTICIPATED HOURS PER WEEK: 28 hrs per week

Minimum Education and Experience Requirements:

Completed Bachelor’s Degree in a human services field and specific work experience/education/training in mental health.

Preferred Qualifications:

Bachelor of Social Work degree or one year of experience providing social work or case management services in a mental health setting.

Description of Duties:

This Case Manager position works within the Student Life Division and Counseling Services and reports to the (DUR) Dean of Counseling or (MOR) Director of Counseling and Wellness and the Director of Campus Health and Safety. The duties of the position include:

Review counseling and medical forms prior to students’ arrival to coordinate needed services and create appropriate Student Life support plans with families and students

Referral coordination to medical providers and mental health providers

Assist students with referrals to the UNC System Office telepsychiatry service pilot program

Follow-up with students and parents if students miss their medication pick-up with clinic staff

Follow up with parents and students regarding excused medical and counseling-related absences and create support plans

Work with clinicians, counseling team members, and medical personnel to create appropriate individualized plans to support the mental and physical needs of the student;

Assist parents and students in finding additional needed resources in the community

Maintain an up-to-date database of referral agencies and individual providers in the local area and establish and maintain relationships with local mental health providers

Assist students in their transition to higher education. This includes developing a plan and connecting students with colleges’ mental and medical health care services with the goal of decreasing instances of disruption in critical services and improving continuity of care during the transition.

Other case management duties as assigned.

Description of Responsibilities:

Client-related services: 70% (Meeting with students, case consultations with students, families, and medical and mental health providers, collateral contacts with family and guardian or other supports, transportation coordination to medical and mental health appointments, and secure resources related to students’ wellness)

Meetings: 10% (Staff meetings with Counseling Services, Health Services staff; IAP or ISP meetings, and other related committee meetings)

Administrative Duties: 10% (Timely record keeping; compiling reports, development of resources and referral database, and completing documentation related to referral services and NCSSM and grant-funded mental health services)

Community engagement: 10% (Develop community partnerships for internal and external referrals and opportunities (virtual and in-person); meetings with community service groups

Knowledge, Skills, and Abilities:

Knowledge: The position requires knowledge of basic medical social work case management principles and strategies, adolescent mental health and wellness with best practices, and skills in the assessment and monitoring of student health.

Consulting: The position requires the ability to communicate information to students, parents, and external providers. The ability to collaborate with counseling staff, clinic medical staff, and NCSSM’s administration is required.

Critical Thinking: The position requires the basic ability to make informed clinical decisions, bringing in appropriate consultation with other professionals when necessary. The position also needs to be able to organize and analyze information to improve the quality of care and determine what information is relevant to improve outcomes.
