Neuro Community Care LLC is a fast-growing, independent Case Management provider, specializing in community-based support for persons with traumatic brain injury, spinal cord injury and other neurological conditions.

Our organization is seeking a full-time Brain Injury Case Manager, who will identify, locate and provide community supports and resources to clients in order to reintegrate and reconnect with their communities and to increase their independence.
Essential Functions
• Review the case file and complete an assessment to determine the needs, goals, and interests of the client.
• Develop individualized goals that outline functional supports to acquire, retain, and improve independence.
• Research, connect, and communicate with local providers focusing on Social Determinants of Health to support clients’ goals.
• Work with Community Support Specialist (CSS) or other local service provider staff to implement identified services to meet the client’s goals.
• Work and communicate effectively with client, families, NCC personnel, referral sources, and others.
• Maintain confidentiality, while communicating to necessary parties to evaluate progress and re-evaluate goals.
• Complete necessary documentation in compliance with organization policies.
• Responsible for monitoring and appropriately utilizing billable hours with clients.
• Provide positive support and be an advocate for the client.
• Identify barriers, triggers, or changes in functional status to develop proactive interventions that promote continued independence.
• Participate in team meetings, conferences, workshops, and trainings to enhance job skills and development.
• Position requires local and domestic travel, either by plane or car. Some out of area or overnight travel should be expected.

-Bachelor’s Degree in related field required(Social work, mental health, speech-language pathology, psychology, recreation therapy or other human services); Master’s Degree preferred,
-Ability to achieve and maintain certification as a Brain Injury Specialist (within 1 year).
-Must have a valid Driver’s License, as well ensure vehicle is compliant with NC DMV standards.
-CPR and First Aid certification (can acquire).
-Ability to travel throughout the country and work in an office environment.
-This position requires normal periods of sitting and standing in an office environment, lifting and/or pushing objects up to 25 lbs.
