This position operates as a behavioral health consultant (BHC) on the medical team utilizing the Primary Care Behavioral Health (PCBH) model of care. Acting as a generalist, the BHC sees patients of all ages and with wide ranging medical, behavioral health and behaviorally influenced conditions. As a fully integrated member of the primary care team, the BHC provides episodic behavioral health services and interventions to a high volume of patients.

The BHC’s additional responsibilities will include teaching residents, faculty, patients, families, and staff care, prevention, and treatment enhancement techniques, monitor behavioral health program outcomes, identifying problems related to patient services and making recommendations for improvement, participate in development and administration of data and measures to assess multiple outcomes, to include patient and provider satisfaction, institutional and provider fidelity to the model, patient health outcomes, etc.

This is a partially grant-funded position. The BHC will be a primary member of the grant team and serve as a teacher to resident physicians who will work alongside the BHC during their clinical rotations within the Patient Center Behavioral Health (PCBH) model of integrated care. The BHC will serve as a subject matter expert for medical practitioners and provide real-time education and feedback for Family Medicine residents on their screening, assessment and treatment skills. The BHC will support Family Medicine residents and faculty in building referral and interprofessional care skills, as well as independence around screening, assessment, and treatment of mental health issues.
