CANDIDATE REQUIREMENTS:
Licensed in North Carolina as LCMHC, LCSW, or LMFT or license eligible
Provisionally licensure counselors are encouraged to apply LCSWA
ESSENTIAL JOB FUNCTIONS:
Providing and coordinating the assessment and reassessment of the recipient’s clinical needs.
Perform clinical face-to-face outpatient therapy services in either group or individual setting.
Perform intensive clinical and functional face-to-face evaluations of individuals with mental health, developmental disability, or substance abuse condition and determine if the individual meets target population criteria.
Assure clinically appropriate services are delivered to eligible recipients within the service definition (right person, right treatment, right intensity, frequency, and duration)
Completing the initial development and ongoing revision of the Outpatient Treatment Plan or Person Centered Plan and ensuring its implementation.
Consulting with identified medical (for example, primary care and psychiatric) and non-medical (for example, DSS, school, DJJDP) providers, engaging community and natural support, and including their input.
Ensuring linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
Ensure that client information is kept confidential.
Monitoring and documenting the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the Person Centered Plan.
Provides feedback to the Clinical Director and Medical Director regarding services rendered.
