Aging Transitions Specialist

General Description
DISTINGUISHING FEATURES OF THE CLASS
The work involves the responsibility for providing professional-level eldercare services to older adults and their caregivers and providing task supervision of the work of undergraduate and graduate level human services students and interns. An employee in this class performs clinical and administrative work including assessments, care planning and counseling, service coordination, advocacy, and community education for persons dealing with dementia, Parkinson’s disease, and other chronic conditions with the aging population. Employees may be assigned to work with various divisions within the Department on Aging, including Aging Transitions and Volunteer Connect 55+.

Work is performed under the general supervision of the Aging Transitions Administrator and the Volunteer Connect 55+ Administrator with some leeway to exercise independent judgment within the framework of applicable rules, regulations, policies and procedures in the performance of duties, both in terms of planning and accomplishing work and in making decisions about public health needs and requests. Supervision is a responsibility of this class. Does related work as required. This position supervises graduate level interns and volunteers.

MINIMUM QUALIFICATIONS
Graduation from an accredited college or university with a bachelor’s degree in a human service field AND two (2) years of work experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment planning; OR

An equivalent combination of training and experience as defined above. A higher degree does not substitute for work experience.

Master’s degree in social work and two (2) or more years of experience working with the aging population is preferred and required for providing independent supervision if applicable.

Bi-lingual in Mandarin is preferred.

SPECIAL REQUIREMENTS
All offers of employment are conditional upon successful clearance of a background check. Employment cannot commence until the employee has been cleared.

Possession of or ability to obtain CPR and First Aid certification within 6 months of date of hire preferred.

May be required to possess additional certification(s) as deemed necessary by the Aging Transitions Administrator.
