Engagement Specialist

Housing for New Hope prevents and ends homelessness by providing access to housing and healthcare and integrated services in the Durham community. We currently have an opening for an Engagement Specialist within our Assertive Engagement Team.

 

Position Summary:  Under the direction of the Assertive Engagement Program Manager, the Engagement Specialist will participate in outreach activities to individuals who are homeless and experiencing mental illness and other medical impairments.  The Engagement Specialist will function within a team environment to include on-going assessment, support and crisis intervention.

 

Reports to: Assertive Engagement Program Manager

 

Responsibilities:

  • Engage and participate in outreach activities to individuals living on the streets, parks, encampments, abandoned buildings, etc. who are identified as experiencing homelessness and/or chronic homelessness
  • Provide clinical case management services to assigned caseload
  • Conduct client interviews and perform clinical assessments; make referrals and consult with other service providers (i.e. mental health, primary care, substance use disorder, wellness recovery, etc.)
  • Develop and modify individual person-centered plans to assess on-going needs for clinical supportive services and related housing needs
  • Conduct initial housing screening using the identified SPDAT; provide the most appropriate level of service by prioritizing need
  • Assist clients to modify attitudes and patterns of behavior by increasing understanding of self and barriers; utilize motivational interviewing techniques
  • Maintain thorough and accurate case notes, files, and correspondence while maintaining confidentiality of clients
  • Ensure completion of monthly statistics and program records, including HMIS
  • Responsible for case staffing and clinical treatment reviews with team
  • Coordinate clients’ move-in dates to permanent supportive housing units; provide limited clinical support to tenants residing in scattered sites;
  • Work collaboratively with internal and external multi-disciplinary teams (i. e. care coordinators, clinical medical staff, property management, etc.) to mitigate clients’ issues, including housing challenges
  • Provide on-call coverage, resulting in crisis intervention and resolutions as needed
  • Participate in community meetings and other activities
  • Attend workshops, trainings, and webinars as necessary to increase skills and knowledge to provide effective support
  • Abide by and maintain compliance with agency’s policies and procedures
  • Other duties as assigned.

Qualifications:

  • Bachelor’s degree in human services and related fields
  • Three (3) years of relevant experience related to working with individuals diagnosed with a behavioral health disorder
  • Three (3) years of relevant experience related to providing case management services to individuals experiencing homelessness or chronic homelessness
  • HMIS experience preferred
  • Significant knowledge of community and supportive housing resources, the work first model, Fair Housing Act, and Americans with Disabilities Act (ADA)
  • Ability to work with diverse communities.
