This position provides intensive case management and supportive services to the aging and disabled population for individuals to remain safely in their homes for as long as possible or to reside safely in a facility. This position could be assigned tasks associated with various services and programs in the Adult Services unit.

Responsibilities include:
Evaluating reports of alleged abuse, neglect and/or exploitation of disabled adults under the provision of Adult Protective Services as well as serving as case manager for individuals deemed incompetent by the court and assigned to the agency for guardianship.

Case management includes arranging and coordinating an array of services, the comprehensive completion of quarterly and annual assessments to determine services needed, accurate and timely documentation, collaboration and cooperation with other agencies and monitoring the safety and well-being of older and/or disabled adults.

Attends court proceedings and work collaboratively with the Agency legal staff to draft petitions, prepare court reports and necessary documents; and testify in court in support of the Agency position in legal matters.

Must often work with complex and challenging family dynamics, acute to severe medical and/or emotional disorders and mental illnesses that the client may possess as well as handle emergencies and crises as they arise-on an almost daily basis.

Will participate in the rotation of on-call duties with other staff to receive and respond to reports involving allegations of adult abuse, neglect and/or exploitation during after-hours and weekends.
Participate in shelter operations during a disaster or weather-related incident.

Minimum Qualifications:
Master’s degree from an accredited school of social work and one year of social work or counseling experience; or a bachelor’s degree from an accredited school of social work and two years of social work or counseling experience; or a master’s degree in a counseling field and two years of social work or counseling; or four year degree in a human service field or related curriculum including at least 15 semester hours in courses related to social work or counseling and three years of social work or counseling experience; or graduation from a four year college or university and four years of experience in rehabilitation counseling, pastoral counseling, or a related human services field providing experience in the techniques of casework, group work, or community organization; or an equivalent combination of training and experience.

Awareness of needs and challenges associated with the aging and disabled population and the ability to navigate community resources is strongly desired.

Salary:
Grade 63: $48,519
