Wake Forest Baptist Health is seeking a licensed clinical social worker for Internal Medicine/Gerontology’s Memory Counseling Program to provide social work services, clinical intervention, and care coordination to patients with dementia and their families. Experience with older adults and/or patients and caregivers with dementia preferred. Clinical interventions include conducting family meetings and individual, family, and group counseling with patients and families who present with a variety of behavioral health needs in the context of a dementia diagnosis in the family. The LCSW will assist with the development of a comprehensive treatment plan to meet ongoing health and mental health needs and may also coordinate with other community service providers.

Master of Social Work from an institution accredited by the Council on Social Work Education is required, plus clinical licensure in the state of North Carolina through the Social Work Certification and Licensure Board. Clinical social work experience required. Minimum three years’ experience working in a mental health setting with clinical diagnosis and treatment responsibilities.
