This position is equally responsible for providing comprehensive substance use treatment and medical case management, mental health, and referral services to adults living with HIV infection receiving medical services in the UNC Infections Diseases Clinic. A much smaller percentage of time will involve provision of services to patients with other infectious diseases in collaboration with members of the medical and social work teams.
This position is responsible for coordination, planning, and implementation of the substance use treatment plan including a wide range of patient centered services that facilitate linkage to and retention in HIV and associated medical care. Services may be delivered in individual and/or group settings. Services may include but are not be limited to comprehensive psychosocial and substance use assessment, medical case management, mental health and counseling services, assistance with transportation, housing, nutrition, financial or other needs. Substance Use Treatment includes pretreatment/recovery readiness, harm reduction, behavioral counseling, linkage to medication assisted therapy and neuro-psychiatric pharmaceuticals, and relapse prevention. Substance Use counselor will implement an SBIRT model in ID clinic for identifying and providing effective interventions and referrals for patients at risk for substance use disorders.
This position will work closely with medical providers and other interdisciplinary team members including physicians, advance practice providers, nurses, social workers, pharmacists, financial counselors and others.
Employee must be able to collaborate with other community and social service agencies to facilitate services for patients. They will play an active role in program development and clinical quality improvement initiatives in the clinic. Employee must have ability to effectively communicate with patients, their families and support system to promote compliance with treatment plans directed. This position involves utilizing all types of communication, such as a variety of platforms (EMR communication, texting, email, phone calls, etc.) in communicating with faculty, staff, patients, their support system, and their families to promote achievement of treatment plan goals.
