SUMMARY: Works in collaboration with the primary healthcare team providing behavioral healthcare services within scope of practice. Provides screening, assessment, diagnoses, evaluation, crisis intervention, treatment, consultation, care coordination and case management, education, referral, advocacy, and other related outpatient services.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
1. Serves as the Behavioral Health Expert and provides leadership in area of clinical expertise for the organization in the Person County school system.
2. Provides/conducts behavioral health screening, assessment, diagnostic, consultative, referral, case management/care coordination and evaluation services. Both at the agency and in the Person County school system. Must be able to manage a schedule to accommodate various locations.
3. Provides crisis intervention services and serves as emergency contact for behavioral health issues in the school system and determines what if any other agency should be called, like mobile crisis or the intensive-in-home team.
4. Provides individual, family, group, and other counseling services at the agency and in the school system by working with each individual school.
5. Develops and implements patient centered treatment plans including support of the primary care/comprehensive treatment plan for all clients
6. Maintains clear, accurate, and timely documentation and record keeping.
7. Maintains compliance with local, state, federal standards/rules as well as organizational mission, values, policy and procedures.
8. Maintains current knowledge of behavioral health issues, impact on practice, and best practices.
9. Participates in quality improvement, utilization review and peer review processes.
10. Serves as part of the medical home healthcare team, provides collaborative and consultative care with the Primary Care Provider as well as with other members of the team with participation in team huddles and treatment planning meetings as needed.
11. Compliance with workflows, policies and procedures as well as other activities to support PCMH model of care and Joint Commission accreditation.
12. Participates in various committees within and outside of the agency related to behavioral health issues especially as it relates to the Person County school system.
13. Performs other necessary duties as required to meet the goal of providing behavioral health care services.
14. Provides training in the Person County school system by working with the individual schools and the coordinator of counseling services for the school system.
15. Help coordinate with the School Manager the protocols needed to engage a student in the school system with this agency. Make sure that the proper paperwork needed is completed and given to the staff to be entered into the medical records software.

QUALIFICATION REQUIREMENTS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

EDUCATION and/or EXPERIENCE:
Masters Degree of Social Work and North Carolina Clinical Social Work Licensure (LCSW) is required. Two years of supervised social work experience as required by the North Carolina Social Work Certification and Licensure Board.
Knowledge of public/private behavioral health treatment delivery systems.

LANGUAGE SKILLS:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to communicate with patients and family members of various educational, socio-economic and cultural backgrounds.

MATHEMATICAL SKILLS:
Ability to apply concepts of basic mathematics

REASONING ABILITY:
Ability to participate in problem solving and implement appropriate steps for problem resolution under the Medical Director’s supervision. Ability to carry out a variety of instructions furnished in written, oral or diagram form. Ability to exercise sound judgement.

COMPETENCY/SKILLS REQUIREMENTS:
Competency required in the areas of communication, excellent verbal and written skills, and ability to provide individual, group and family therapy with the ability to organize work and meet objectives timely and efficiently. CPR certified.

CERTIFICATES, LICENSES, REGISTRATIONS:
Current North Carolina Clinical Social Work Licensure and valid North Carolina Drivers License. Must have a current North Carolina Driver’s license.
