The Department of Psychiatry at the University of North Carolina at Chapel Hill School of Medicine is seeking a social worker to assist with patient-facing referral management and initial patient assessments for the Comprehensive Cancer Support Program (CCSP)‘s psycho-oncology clinical service. This individual will assist with the expansion of current care pathways to better meet the needs of a diverse patient population. This is a 100% FTE, fixed-term Clinical Instructor.
The social worker will fulfill a critical role in the overall clinical care of patients being treated for cancer at the NC Cancer Hospital and their family members. The principal responsibility of this position will be to assist with referral outreach, clinical team communication, improving clinical workflow pathways, and conducting initial and ongoing supportive care needs assessment, with the goal of improving access and care delivery for patients and CCSP clinicians.
The successful candidate will conduct brief initial needs assessments and facilitate subsequent care coordination for patients referred to the CCSP psycho-oncology service. Additionally, the social worker will work closely with CCSP leadership to develop a roadmap to ensure more equitable and culturally adapted and appropriate care for patients by critically examining current pathways and access to services. Additional duties include informing patients about additional CCSP services and making appropriate referrals to these additional services as needed. This position will be organizationally housed within the CCSP and will report to the Director of the CCSP’s psycho-oncology clinical service. Supervision by a licensed clinical social worker will be available if the social worker is interested in future licensure.
