JOB SUMMARY: The Case Manager provides a range of comprehensive community-based case management services, including housing services and trauma-informed case management for individuals who are homeless and/or returning to the community from incarceration. The position serves as a liaison to the District of Columbia Department of Corrections, READY Center, Anchor Mental Health (Core Service Agency), and Housing, to identify and coordinate community resources and make the appropriate referrals. The Case Manager establishes and maintains effective working relationships among area residents, property owners in the community.

ESSENTIAL DUTIES and RESPONSIBILITIES:

Interview and assess the needs for direct services of Returning Citizens (post-release) and individuals referred by jails or prisons including the DC Jail prior to discharge (pre-release), primarily in the District of Columbia, who have mental health needs and/or a history of addictive behaviors.
Perform a Vulnerability Assessment Test on all participants entering the program. Work closely with Program Manager and Employment Case Manager to identify clients who meet admission criteria.
Develop individualized goal plans and tasks that guide participants to resolve specified needs (mental health/substance abuse/co-occurring disorders, employment, housing, health care, vocational, educational) throughout their entire participation in the program.
Work closely with Employment Case Manager, Substance Abuse Education Coordinator, Anchor Mental Health staff, and other community partners to identify and help clients overcome risk factors, including anti-social attitudes, peers, and/or behaviors, as well as family and marital problems, school and work issues, and/or substance abuse or co-occurring disorders.
Maintain complete and accurate case records to include entering data in the database within 48 hours of contact/service.
Report progress and accomplishments to the Office of Victim Services and Justice Grants (OVSJG).
EDUCATION and EXPERIENCE:

Bachelor’s degree in social work, human services, or related field.
For social work degree – social work licensure in the appropriate jurisdiction is required.
3 years of experience working with incarcerated adults including those with mental disorders.
Must be able to pass a background check to gain access to secured facilities.
SKILLS and COMPETENCIES:

Ability to communicate effectively in written and oral form and establish and maintain cooperative working relationships.
Ability to work under pressure and prioritize assignments.
