This position will serve in the role of Qualified Professional (QP) on the UNC-Wake County Assertive Community Treatment Team (ACTT). ACTT consists of professionals from multiple disciplines (psychiatry, nursing, social work, and rehabilitation counseling) who use a team approach to meet the needs of individuals with severe and persistent mental illness, as well as co-morbid developmental disabilities or substance abuse disorders, in community-based settings. ACTT works according to a recovery-based philosophy and provides person-centered services addressing the breadth of an individual’s needs, helping him or her achieve their personal goals. Thus, ACTT is generally the first-responder and single point of responsibility for meeting the client’s needs, which necessitates higher frequency and intensity of services and a low staff-to-client ratio. This position will be responsible for facilitating the person-centered planning process for assigned individuals, as well as carrying out a range of psychosocial rehabilitative interventions and case management tasks. Area(s) of focus may be determined by needs of the client population and could include housing, criminal justice, employment, and psychiatric rehabilitation.
