Behavioral Health Care Manager

We are seeking a behavioral health care manager (BHCM) who will be responsible for supporting
and coordinating the mental and physical health care of patients on an assigned patient caseload
with the patient’s primary care provider (PCP) and, when appropriate, other mental health
providers. The BHCM will be fully integrated in our office as part of the Collaborative Care Model
(CoCM), and function as a member of an interdisciplinary team, which includes the patient’s PCP,
mental health therapists, consulting psychiatrist, as well as the larger primary care team. The
BHCM provides screening, brief evidenced-based treatments, and care coordination for our
patients and their families. When available, the BHCM can be a consultant for the PCPs to meet
with patients in the medical exam room upon a warm hand off from the PCP. We offer a

hours of Monday through Friday 8am-5pm. There are no weekends or evenings required, no
on-call responsibilities, and no hospital work.
Applicants required to have one of more of these North Carolina licenses: Licensed Clinical
Mental Health Counselor (LCMHC), Licensed Clinical Social Worker (LCSW), Licensed Marriage
and Family Therapist (LMFT), or Registered Nurse (BSN recommended) with mental health
experience.
