We are currently seeking a full-time Care Manager (QP status required) for our new Tailored Care Management Team. The primary role of the Care Manager is to support and coordinate the mental and physical health care of identified members that are appropriate for Tailored Care Management Services. The TCM Care Manager will work closely with the TCM Supervisor and TCM Extender (Peer Support Specialist) to ensure a fully supported work environment.

The Tailored Care Management model reflects the goal of whole-person care management. Provider-based care management promotes integrated care and offers beneficiaries choice in how they receive care management.

About SouthLight Healthcare

Founded in 1970, SouthLight Healthcare is one of the area’s largest and long-established nonprofit providers of substance use treatment and mental health services. With outpatient and community-based programs, SouthLight provides prevention, education and treatment services in the Triangle and beyond. SouthLight has multiple office locations throughout the Triangle area, serving more than 9,000 clients each year. Services include adult outpatient counseling, Intensive Outpatient Program, Psychiatric Services and Medication Management, Opioid Treatment Program, Court Deferral Program, Peer Support Services, Family Centered Treatment, Community Support Team and a residential treatment program for women and children and more.

JOB RESPONSIBILITIES

Screens and assesses members for common mental health and substance use disorders.
Facilitates member engagement and follow-up care.
Provides member education about common mental health and substance use disorders and the available treatment options.
Provides appropriate interventions to members under the supervision of the Tailored Care Management Supervisor
Provides case management services as necessary to tailored plan members
Provides assertive engagement to appropriate members engaged in Tailored Care Management services
Completes documentation in a timely manner.

QUALIFICATIONS

Educational and Experience

Bachelor’s Degree in relevant field
Minimum of 3 years’ experience providing case management or care coordination with adult MH/SA population in a community setting.
Licensure/Certifications

QP status required as specified in NC statue 10A NCAC 27G .0104
Valid driver’s license with good driving record
Full or Provisional License MH/SA (LCMHC/A, LCAS/A, LMFT/A, LCSW/A) or CADC preferred but not required
Knowledge, Skills and Abilities Required

Excellent organization and time management skills
Maintain flexible and coachable demeanor
Ability to develop rapport with members and other professionals in a variety of settings
Ability to set firm boundaries with members
Excellent clinical skills including: evidence-based practices, diagnostic information (DSM 5), substance use intervention, etc.
Proficient in Microsoft Office Suite, Outlook, and electronic medical records.
Excellent interpersonal skills and ability to work collaboratively within a team.
Comfortable working independently in the community
Excellent oral and written communication
Fluent and knowledgeable about Care Management
Ability to manage crisis situations both in person and over the phone
HIPAA and Substance use specific confidentiality and privacy rights

Benefits:

Full-time employees are eligible for medical benefits the first of the month 30 days after hire date. Options for a standard plan and an HSA plan with a $300 initial contribution and up to a $25 per paycheck match. 10 paid holidays and PTO hours which accrue at a rate of 10 hours per month. After 6 months employees are eligible to enroll in our 401(k) plan which has a 4% match. Company paid life insurance, short and long term disability, in house and paid training opportunities.
